Please complete an application so that we can learn more about your child. Applications are required for new students only.

Instructions: Please complete all of the pages of the application.

• Applications submitted by December 31st will receive priority consideration
• A $250/per student fee is due at the time of submission of your application
• For students in Grades 1-8 who advance to an interview, a $250 assessment fee will apply. The fee is waived when a comparable formal cognitive and academic assessment or neuropsychological evaluation completed within the past 12 months is submitted with the application

Please remember that submitting an application does not guarantee the applicant a space or acceptance to Manhattan Day School. Priority admissions are given to current MDS families.
